First Visit
Your initial appointment will consist of a comprehensive dental exam to arrive at your diagnosis and treatment options. Dental cleanings are often performed at initial visits, but this will depend on the information found during your exam. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
We appreciate having the opportunity to prepare your new patient paperwork in advance of your appointment allowing us to focus on your dental needs rather than administrative tasks during your visit. Please email [email protected], mail, or drop off your paperwork (including a list of medications) at least 2 business days prior to your appointment. Please include an image of the front and back of your insurance card(s) (if applicable) and identify primary vs secondary (if applicable). Please notify us if you require antibiotic premedication.
Please help us better serve you by arriving 10-15 minutes early and providing the information above at the time of your consultation if not already provided.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to dental surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken x-rays within the past 5 years, we recommend that you request that they forward them to our office at least 2 business days prior to your appointment. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
